Improvement on treating serious hot-crush hand injuries

摘  要:目的探讨如何改进严重手部热压伤的治疗。方法总结近年来手部热压伤的治疗情况,将22例中、重型热压伤病例按手术方法归纳为带蒂皮瓣、邻近皮瓣、游离皮瓣三组,比较一期愈合效果、总手术次数、及术后并发症情况。结果手部热压伤大多比较严重,需行皮瓣修复,总体修复效果邻近皮瓣和游离皮瓣优于传统带蒂皮瓣。结论手部严重热压伤的治疗创面修复是基础,功能恢复是关键,进一步改善手功能康复效果须着重对手术治疗及其相关的环节加以改进。

[Objective] To explore how to improve the treatment for serious hot-crush hand injuries. [Methods] By summarizing and analyzing the clinical data of hot-crush hand injuries from Jan 1997 to Jun 2005, 22 patients suffered from moderate or severe degree of hot-crush hand injury were sorted out and induced according to each operative method into 3 groups: group A of 6 cases repaired by pedicled abdominal flaps, group B of 6 cases treated by revese forearm flap transfers and group C of 11 cases treated by free fissure flap transplantations, and the results of primary healing, the total times of surgical precedures and the postoperative complications of the cases among different groups were statistically compared. [Results] Serious hot-crushes, the majority of all hot-crush hand injuries should be repaired with verious surgical flaps, and the overall clinical effect of either group B or C was superior to that of group A. [Conclusion] For the treatment of serious hot-crush hand injuries, wound repair could be the basis, while founction reconstruction is the key. To achieve further founctional recovery of hand, a series of innovations upon corresponding links in treatment, especially upon the key link-surgical operations, are essential.
